Understanding Fifth Metatarsal Fractures: Causes, Symptoms, and Treatment Options

Jul 27, 2024

Fifth metatarsal fractures are a common occurrence, particularly affecting the long bone on the outside of the foot connected to the little toe. Dr. Alex Stewart, a member of the American College of Foot and Ankle Surgeons (ACFAS), sheds light on the types, symptoms, diagnosis, and treatment of these fractures to aid in your understanding and recovery.

The two primary types of fifth metatarsal fractures are avulsion fractures and Jones fractures. Avulsion fractures result from an injury where a small piece of bone is pulled off by the peroneus brevis tendon, often occurring alongside ankle sprains. Jones fractures, on the other hand, affect a specific area of the fifth metatarsal base, which is prone to healing difficulties due to reduced blood flow in this area. These fractures can be stress-related or acute and are often caused by overuse or trauma.

Recognizing the symptoms of a fifth metatarsal fracture is crucial. These include pain, swelling, tenderness, difficulty walking, and bruising on the outside of the foot. Prompt diagnosis by a foot and ankle surgeon is essential. Diagnosis involves a thorough examination, including a review of the injury history, physical examination, and imaging studies such as x-rays, ultrasound imaging, and potentially additional scans like bone scans, MRI, or CT scans.

Nonsurgical treatment options are often employed initially, utilizing the RICE method: Rest, Ice, Compression, and Elevation. Immobilization with a cast, cast boot, along with crutches to avoid weight-bearing on the injured foot. Additionally, bone stimulation devices may be utilized, particularly for Jones fractures, to expedite healing.

In cases where the injury is severe, involves displaced bones, or fails to heal adequately with nonsurgical methods, surgery may be necessary. Dr. Alex Stewart and his team determine the most suitable surgical procedure based on individual patient needs. Surgical interventions are typically performed on an outpatient basis at a hospital or surgical center.
